Charges for supplements, prescription medications, other services, and shipping (if applicable) will be
in addition to any consultation, NBT and bloodwork fees.
Outpatient services: Payment is due at the time services are rendered. For your convenience, we accept cash, checks (with identification), *CareCredit, MasterCard, Visa and Amex. *some restrictions apply
Inpatient Services: You will receive a written estimate for all in-patient services prior to admission to the hospital. Fifty percent (50%) of that estimate must be paid at the time of admission and the remainder must be paid in full at the time of discharge. Payment may be made by any of the methods noted above.
Telephone Consultations: A credit card authorization form must be completed and returned to us prior to the date and time of your consultation.
Supplement Orders: Payment is required at the time of pick up, or prior to shipment. For your convenience in placing and processing orders, we will keep a credit card on file for you. We are required to have a signed credit card authorization form in order to keep your credit card information on file.
